Entrada Scholars Supplemental Application

For high school seniors applying to Entrada:
This form is used to apply to the Entrada Scholars Program and to supplement the information provided on the
Calvin University application for admission. If you are a high school senior and want to be considered for the Entrada Scholars Program, you must be admitted to Calvin University and submit this supplemental application for Entrada. Both forms, the Calvin application for admission and the supplemental application for Entrada, are required to determine admission to the Entrada Scholars Program.
